What is the Ohio Change of Address Form?


Form BMV 5756, Ohio Bureau of Motor Vehicles Request for Change of Address, or the Ohio Change of Address Form, is a state-level form by the Ohio Department of Public Safety. Residents of the state of Ohio who are eligible to use it submit it to the said department to change their address on their driver’s license and vehicle registration. Only residents of Ohio and those who are moving to a new residential address within the state are eligible to fill out the Ohio Change of Address Form.


 


The Ohio Change of Address Form also serves as a notification of a change of address for your voter registration records. Signing it provides your consent to the release of the information you supplied to the form to the Ohio Secretary of State’s office. Nevertheless, if you do not wish to release your information to the Ohio Secretary of State’s office or you do not meet the state of Ohio’s voter eligibility requirements, you may opt out to do so.

How to fill out the Ohio Change of Address Form?


To fill out the Ohio Change of Address Form, you will need to supply your personal information and your address details, including your previous and new ones.


 


The Ohio Change of Address Form is a two-page document, but you only need to answer its first page. Complete the form accurately and correctly to avoid any problems or delays with your request to change your address. You must not deliberately provide any false information as doing so is unlawful and may result in penalties or imprisonment.

Correct Residence Address — Enter your correct residence address or where you live or your permanent home address.


New Address


Enter your new address, including the Street, City, State, ZIP Code, County, then specify the name of the city, village, or township.


 


Mailing Address — Enter your mailing address if different from the Residence Address above.


Enter the Street or P.O. Box, City, State, and ZIP Code.

    This section asks you to enter the license plate numbers of the cars in your possession that are under your name.

    Other relevant information you need to need to know when changing an address in Ohio:


  • You may opt to fill out an Ohio DMV Change of Address form at the official Ohio Department of Public Safety. Prepare your current driver’s license number and the last four digits of your Social Security Number.
